Family health team is seeking A compassionate and collaborative Social Worker to join our interdisciplinary primary care team the successful candidate will Mental health and psychosocial support to patients and families within a community based health team setting this role focuses on improving patient well-being through assessment counseling advocacy crisis intervention and connection to community resources
key responsibilities
- Provide assessment and evidence based counseling to individuals couples and families
- support patient experiencing mental health concerns addiction issues grief trauma chronic illness and social challenges
- develop and implement patient centred care plans in collaboration with physicians nurse practitioners and allied health professionals
- conduct crisis intervention and risk assessments as needed
- facilitate referrals to community agencies and specialized services
- advocate for patients and assist with system navigation
- between accurate and timely clinical documentation in the electronic medical record
- participate in team meetings case conferences and quality improvement initiatives
- provide education and support to patients caregivers and community partners
qualifications
- bachelor of social work required, master of social work preferred
- current registration in good standings with the applicable provincial regulatory body Ontario College of social workers and social service workers
- minimum one year clinical experience preferred, ideally in primary care, community health, or mental health settings
- trading an experience in counseling mod alities such as CBT, solution focused therapy, motivational interviewing, or trauma informed care
- strong assessment, communication, and interpersonal skills bilingual is an asset
- ability to work independently and collaboratively within an interdisciplinary team
- experience with electronic medical records considered an asset
- valid drivers license and access to reliable transportation may be require
