Family health team is seeking A compassionate and collaborative registered dietitian to join our interdisciplinary primary care team the successful candidate will provide evidence based nutrition assessment counseling education and chronic disease management services to patients across their lifespan
working closely with physicians nurse practitioners nurses social workers and other allied health professionals the registered dietitian will support patients in achieving their health and Wellness goals through the individualized and group based nutrition care
key responsibilities
- conduct comprehensive nutrition assessments and develop individualized care plans
- provide counseling for chronic disease prevention and management including
- diabetes
- hypertension
- cardiovascular disease
- obesity and weight management
- gastrointestinal conditions
- deliver nutrition education workshops and group programs
- collaborate with interdisciplinary health care providers to support integrated patient care
- document patient encounters in the electronic medical record system
- participate in quality improvement initiatives and health promotion activities
- support culturally safe patient centred care practices
- maintain current knowledge of nutrition guidelines and best practices
qualifications
- registered dietitian in good standings with the applicable provincial regulatory college
- bachelors degree in nutrition diet Ted X or related field
- strong counseling communication and interpersonal skills
- ability to work effectively within an interdisciplinary team environment
- experience with EMR system considered an asset
- proficiency in virtual care delivery is an asset
- excellent verbal and written communication skills bilingual would be an asset
preferred skills and attributes
- motivational interviewing training or experience
- experience facilitating group education sessions
- knowledge of community resources and social determinants of health
- ability to work independently and manage a diverse caseload
