South Centennial Manor is seeking a Nurse Practitioner (NP) to join its Long-Term Care leadership team. This is a highly autonomous, advanced practice role focused on resident-centered medical care, clinical leadership, and hospital avoidance strategies within a collaborative interdisciplinary environment.
Reporting to the Director of Care and working closely with the Medical Director and attending physicians, the NP plays a key role in enhancing quality of life for residents through evidence-based practice, aging-in-place initiatives, and system integration across long-term care, hospital, and community sectors.
The Nurse Practitioner functions as an autonomous advanced-practice clinician, independently assessing, diagnosing, treating, and medically managing residents within legislated scope of practice while providing consistent on-site leadership to strengthen continuity of care, reduce avoidable hospital transfers, and support complex geriatric needs including dementia and responsive behaviours.
Qualifications
- Master’s Degree in Nursing (Nurse Practitioner Program) or BScN with NP certification
- Current registration with the College of Nurses of Ontario in the Extended Class (RN-EC)
- Minimum five years RN experience and two years as an NP in long-term care, geriatrics, primary care, or community settings
- Certification in BCLS/ACLS; GPA, PIECES, Gerontology, or Palliative Care preferred
- Strong clinical judgment, leadership, and collaborative practice skills
- Comfort working autonomously within a complex clinical environment
- Proficiency with electronic medical record systems (e.g., PointClickCare) and Microsoft Office
- Valid Ontario Driver’s License
- A clear vulnerable sector check will be required by the successful candidate as a condition of employment
